About the role

Provides psychosocial services to dialysis patients, supporting their emotional, psychological, and behavioral well-being.

Responsibilities

• Assess patients' psychosocial status and collaborate with the healthcare team to develop care plans.
• Offer supportive counseling, patient education, and assist with transplant referrals and community resource linkage.
• Address treatment adherence, quality of life, and non-adherence issues through interventions and participation in quality reviews.
• Support patients with advance directives, DNR discussions, and navigate insurance and financial assistance processes.
• Assist with patient admission, discharge concerns, and staff training on psychosocial topics.

Requirements

• Master's in Social Work with active state license.
• 2-5 years of related experience.

Skills

• Strong assessment, counseling, and communication skills.
• Ability to collaborate within interdisciplinary teams and facilitate patient-centered care.
• Knowledge of ESRD treatment, community resources, and insurance processes.
