Social Worker - MSW

About the Role

This is an entry-level Social Worker role providing psychosocial services to patients undergoing dialysis. You will work collaboratively with the healthcare team to assess patients' psychosocial status, develop care plans, and offer counseling to promote treatment adherence and improve quality of life.

Supports patients with education on treatment options, transplant referrals, and access to community resources. Addresses barriers related to treatment adherence, insurance, and financial assistance, ensuring comprehensive patient support.

Responsibilities

• Assess and plan patient psychosocial care
• Provide counseling and education to patients and families
• Support treatment adherence and transplant processes
• Facilitate access to community and financial resources
• Participate in quality activities and documentation
• Assist with patient admission and ongoing care coordination
• Collaborate with insurance and billing departments
• Contribute to staff training and team discussions

Requirements

• Master’s in Social Work with a valid state license
• 0-2 years of related experience
• Strong assessment, counseling, and communication skills
• Knowledge of healthcare, dialysis, and community resources
• Ability to work collaboratively within a multidisciplinary team
• Physical ability to perform job functions, including travel if needed
