About the role
Be part of a multidisciplinary team built with compassion and the goal of Moving Health Forward for you and our patients. Work where you matter. Assists in coordinating, planning, implementing and facilitating programs and counseling services on an outpatient basis. Responsibilities may include individual, group and family counseling, crisis services/evaluation, tele-therapy services, and current and future groups/programs. Provides assessments, facilitates treatment and crisis intervention for clientele affected by mental health or dual diagnoses issues. Community/education services and occasional travel are required.
Responsibilities
- Coordinates a clinical treatment program for the diagnosis, treatment or referral of clients/patients presenting moderate to severe social adjustment problems.
- Adheres to policies which assure the confidentiality of client communications.
- Provides for a multi-disciplinary, holistic approach to the treatment of clients, coordinating same with appropriate hospital departments.
- Conducts individual, family, or group therapy sessions in accordance with accepted principles of the National Association of Social Workers.
- Assesses progress of clients/patients and modify or extend treatment program accordingly.
- Coordinates care of clients/patients with referring physician or agency.
- Documents therapy in accordance with hospital policies and procedures on medical records.
- Intervenes as a crisis counselor in situations requiring immediate psychological treatment such as substance abuse, suicide attempts, psychotic episodes and other instances as outlined in hospital policy.
- Refers client or patient to outside agencies where appropriate.
- Communicates with family members/significant others in order to further the effectiveness of client/patient treatment and progress.
- Coordinates client/patient billing and modify treatment program in accordance with insurance coverage, plan provisions, and individual ability to pay.
- Provides educational programs on a variety of psychological topics to hospital/community groups as requested.
Requirements
The individual must be able to work the hours specified. Must have visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.
Qualifications
- Master's degree required.
- Active license in the state of practice upon hire as one of the following:
- Licensed Professional Counselor - Mental Health (LPC-MH)
- Clinical Social Worker - Practitioner in Private Practice (CSW-PIP)
- Licensed Marriage and Family Therapist (LMFT)
- Preferred:
- Active license as one of the following:
- Addiction Counselor Trainee (ACT)
- Certified Addiction Counselor (CAC)
- Licensed Addiction Counselor (LAC)
- 4-6 years related experience and/or training.
- Clinical experience facilitating group therapy sessions and providing individual counseling services.
- Experience working with clients having dual diagnoses issues (addictive disorders in conjunction with mental health conditions).
- Clinical experience with couples and/or family therapy.
- Active license as one of the following:
