Social Worker (MSSP)

About the role

As a Social Worker (MSSP), you will be an integral part of our MSSP - Direct Support team, helping to shape the future of healthcare. You will assess prospective members for various programs, develop, implement, monitor, modify, and document care plans, and accept referrals for case management. This position has been approved to be a Community Worker. If telework is approved, you are required to work within the State of California only, and if partial telework, you must come into the main office in Orange, CA at least two days per week.

Responsibilities

• Participate in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity, and accountability.
• Assist the team in carrying out department responsibilities and collaborate with others to support short- and long-term goals/priorities for the department.
• Provide social work consultation and education to members and families and act as an advocate as needed.
• Analyze assessments to identify individual needs and the resources most appropriate to meet the member's needs.
• Collaborate with an interdisciplinary team to identify and work toward timely resolution of issues related to psychosocial needs, including assistance with financial issues, transportation, and community support services.
• Assist in the coordination of the member's identified psychosocial needs, utilizing community resources and support when appropriate.
• Work with external utilization management personnel and the community in identifying members who could benefit from case management services and to actualize service plans.
• Monitor all services provided to members and ensure necessary and available resources are being utilized.
• Evaluate and anticipate members' needs, provide support, and maintain a role as the members' liaison while forecasting independence and decision-making on their part.
• Provide referrals based on the members' assessed needs.
• Provide ongoing evaluation of the members' progress, the effectiveness of the service plan, and the efficacy and appropriateness of the services provided.
• Partner and communicate with members, family members, significant others, physicians, and other healthcare providers to accomplish goals identified on the care plan.
• Initiate appropriate follow-up care and develop individual care management services/care plans incorporating assessment, education, resource planning, and coordination of services for members accepted for case management.
• Accept referrals for case management and serve as CalOptima Health's liaison to community agencies, organizations, and State of California personnel.
• Maintain documentation of case management plans/interventions and statistics required to demonstrate the impact of case management on quality, cost-effective care.
• Initiate case conferences as needed.
• Develop and maintain a network of current community resources and services where members can be referred for assistance.
• Complete other projects and duties as assigned.

Requirements

• Master's degree in social work plus 2 years of experience working with the gerontology population; an equivalent combination of education and experience may also be qualifying.
• Experience with behavioral health, health facility, health plans, counseling, case management, home health, or hospice.
• Valid driver's license and vehicle, an acceptable driving record, and current auto insurance or other approved means of transportation for work away from the primary office 50% of the time or more.

Qualifications

• Bilingual in English and one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Russian, Spanish, Vietnamese).

• Ability to visually read information from computer screens, forms, and other printed materials.
• Ability to speak (enunciate) clearly in conversation and general communication.
• Hearing ability for verbal communication/conversation/responses via telephone, telephone systems, and face-to-face interactions.
• Manual dexterity for typing, writing, standing and reaching, flexibility, body movement for bending, crouching, walking, kneeling, and prolonged sitting.
• Lifting and moving objects, patients, and/or equipment 10 to 25 pounds.

• Competitive compensation: Pay Grade 309 - $68,015 - $108,824 ($32.70 - $52.3192 per hour). The final salary offered will be based on education, job-related knowledge and experience, skills relevant to the role, and internal equity.

Schedule

This position may require a flexible schedule, including availability to participate in evening and weekend events.

Work Environment

Work is typically indoors and sedentary, subject to schedule changes and/or variable work hours, with travel as needed. Depending on the location:

• If at the 500, 505 Building or a remote work location: No harmful environmental conditions; noise level is usually moderate.
• If at PACE: Indoors in a clinical setting serving the frail and elderly; may have harmful or hazardous environmental conditions; noise level is usually moderate to loud.
• If in the community: Indoors and sedentary, subject to schedule changes and variable work hours with travel; occasional outdoor work in varied temperatures; may have harmful or hazardous environmental conditions; noise level is usually moderate to loud.
