Social Worker (MSW)
About the Role
As a hospice medical social worker, you will be responsible for psychosocial evaluations and ongoing counseling of patients and families during their end-of-life journey. Working in accordance with the plan of care, you will provide emotional support to patients and families when it's needed most.
Responsibilities
- Conduct psychosocial evaluations of patients and families
- Provide ongoing counseling and emotional support during the end-of-life journey
- Work in accordance with the established plan of care
Qualifications
- MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
- Current state license as a social worker
- 2+ years of clinical work experience, preferably in healthcare or hospice
- Reliable transportation
- Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs
- Ability to bear the weight of an average adult effectively
