About the Role

Provide Personalized Recovery Oriented Services to program participants, including individual and group counseling as well as case management and coordination of services. Conduct assessments, develop Individualized Recovery Plans, and maintain documentation of clients’ participation and progress.

Responsibilities

  • Responsible for timely and accurate documentation and follow-up for assigned caseload.
  • Maintain a caseload of individuals for engagement, assessment, rehabilitation planning, and goal setting/acquisition.
  • Maintain and update all records for assigned caseload.
  • Collaboratively document progress notes that include assessment of progress and review of IAP/IRP.
  • Act as leader/facilitator of classes and activities for program participants; develop curricula and group/class protocols as necessary.
  • Participate in IPS (Individual Placement and Support), IRGA, and/or IDDT team meetings.
  • Provide and initiate job development, job placement, and community outreach as needed.
  • Communicate information with all staff and director/supervisor(s) verbally and in writing.
  • Complete referral process for individuals as needed; participate in discharge planning, including initiation of necessary referrals.
  • Provide individual and group rehabilitation/recovery guidance to PROS community members.
  • Report safety issues or concerns; report participant or program issues to the Program Director.
  • Participate in evacuations and ensure program participant safety in emergency situations (e.g., fire, fire drills).
  • Work to achieve productivity standards set by administration.
  • Supervise student interns under the direction of Program Supervisor/Coordinator/Director.
  • Handle incident reporting when applicable.
  • Comply with all Program, WellLife Network, and OMH policies and procedures, including participant rights, documentation, and incident reporting.
  • Adhere to OMH guidance documents and PROS standards of care.
  • Ensure compliance with HIPAA and confidentiality guidelines; uphold confidentiality.
  • For licensed/credentialed professionals, adhere to standards of care set by NYS Office of Professions and professional code of ethics.
  • Complete all required educational training as directed by one’s profession.
  • Assist and empower individuals to identify and achieve recovery goals related to employment, education, and community integration.
  • Develop and implement classes, groups, and activities within PROS categories: IR, IDDT, CRS, and CT.
  • Provide services in specialized teams such as Intensive Rehabilitation Goal Acquisition (IRGA), Individual Placement & Support (IPS), Co-occurring Disorders (IDDT), Community Resource Development, structured skills development, or wellness self-management.
  • Participate in individual, group, family, and other collateral meetings on and off-site.
  • Be proactive regarding case reviews as needed.
  • Communicate with assigned prescribers for participants on caseload.
  • Act as liaison with other agencies to provide information about the PROS program and facilitate new referrals.
  • Coordinate treatment/recovery team meetings for individuals within the program.
  • Provide coverage for programming or classes usually facilitated by other staff as needed.
  • Attend and participate in regularly scheduled team meetings, in-service trainings, and supervision.
  • Attend trainings and conferences as needed.
  • Function as a vital part of an interdisciplinary team environment.

Requirements

  • Minimum of a Master’s Degree in Social Work, Mental Health Counseling, or Rehabilitation; or Bachelor’s Degree in Social Work, Mental Health Counseling, or other human service area with two to three years of mental health/human services experience working with individuals with disabilities.
  • Experience working with individuals with mental health conditions, both individually and in group settings, is required.
