Social Worker - Level IV

About the Role

This class of positions encompasses professional, supervisory, or administrative work of varying degrees of difficulty and responsibility in the operation of a social work department providing psychosocial and other social work services to different types of patients across various settings. Ensures patient-centered quality care and services in support of better outcomes and improved patient experience, in accordance with all applicable local, state, and federal rules and regulations, and social work standards of care.

Responsibilities

• Performs and supervises psychosocial evaluations through interviews with patients and/or identified surrogates, formulating appropriate social work treatment plans.
• Provides individual and group counseling, as well as concrete social services, within healthcare or community settings in NYC.
• Consults with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care, including developing treatment plans, evaluating progress, and creating discharge and aftercare plans.
• Provides outreach services, makes referrals to governmental, public, and/or community agencies, and prepares patients to accept services.
• Documents and reports suspected cases of child abuse or neglect to the New York State Central Registry as a mandated reporter.
• Appears and testifies at administrative and court hearings, as necessary.
• Assists in planning for institutional or hospital discharge and evaluates alternate care arrangements for patients.
• Participates in staff conferences, committees, research projects, and quality assurance/performance improvement activities.
• Prepares reports, keeps appropriate records, and ensures compliance with NYC Health + Hospitals policies, national standards, and regulatory requirements.
• Mentors less experienced social work staff and serves as a resource in advanced social work practice.
• Supervises graduate and undergraduate social work students, volunteers, and auxiliary personnel.
• Provides direct clinical supervision and guidance to social work staff, assigns cases, schedules staff, and evaluates work performance.
• Maintains individualized supervision logs and ensures timely completion of attestation forms for LCSW licensure applications.
• Conducts or leads special studies and projects related to vulnerable patient populations, preparing and presenting reports.
• Establishes and maintains professional social work standards of performance for interview techniques, clinical assessments, and case documentation.
• Interprets clinical information and prepares comprehensive reports supporting recommended courses of action.
• Develops training curricula and educates other professionals on social work concepts and functions.
• Trains staff in agency policies, procedures, and appropriate social work techniques.
• Ensures compliance with accepted social work practices, government rules, and regulations.
• Participates in conferences, committees, and meetings with supervisory staff, clinical professionals, and community representatives.
• Analyzes population health issues and serves as a liaison to community agencies.
• Provides clinical supervision and professional expertise to multiple social work units on complex cases.
• Coordinates staff work with other agencies to enhance patient experience and achieve mutual objectives.
• Plans, develops, implements, and coordinates programs and activities to meet identified needs.
• Provides consultation and technical assistance, assessing staff training needs and conducting training sessions.
• Interprets and communicates procedures, regulations, and program directives.
• Responds to inquiries from patients, agencies, and courts, and attends interdisciplinary meetings.
• May draft policy and procedural manuals or other instructional guides for approval.

Requirements

• Valid license as a Licensed Clinical Social Worker (LCSW) issued by the New York State Education Department (NYSED).
• One (1) year of supervisory experience utilizing casework, group work, and/or community organization methodologies.
