Social Worker Level IV - ACT

About the Role

This class of positions encompasses professional, supervisory, or administrative work of varying degrees of difficulty and responsibility in the operation of a social work department providing psychosocial and other social work services to different types of patients across various settings. Ensures patient-centered quality care and services in support of better outcomes and improved patient experience, in accordance with all applicable local, state, and federal rules and regulations, and social work standards of care.

Responsibilities

• Performs and supervises psychosocial evaluations through interviews with patients and/or identified surrogates, formulating appropriate social work treatment plans.
• Provides individual and group counseling, as well as concrete social services, within health care or community settings.
• Consults with professionals in other disciplines to determine and implement a multidisciplinary approach to patient care, developing treatment plans, evaluating progress, and creating discharge and aftercare plans.
• Provides outreach services, makes referrals to governmental, public, and/or community agencies, and prepares patients to accept services.
• Documents and reports suspected cases of child abuse or neglect to the New York State Central Registry as a mandated reporter.
• Appears and testifies at administrative and court hearings, as necessary.
• Assists in planning for institutional or hospital discharge and other post-institutional care, evaluating alternate care arrangements.
• Participates in staff conferences, committees, research projects, and quality assurance/performance improvement activities.
• Prepares reports, maintains records, and ensures compliance with NYC Health + Hospitals policies, national standards, and regulatory requirements.
• Mentors less experienced social work staff and serves as a resource in advanced social work practice.
• Supervises graduate and undergraduate social work students, volunteers, and auxiliary personnel.
• Provides direct clinical supervision and guidance to social work staff, including case assignment, scheduling, and performance evaluation.
• As a Licensed Clinical Social Worker (LCSW), provides and documents clinical supervision of Licensed Master Social Workers (LMSWs) toward advanced licensure, maintaining supervision logs and attestation forms.
• Conducts or leads special studies and projects related to vulnerable patient populations, preparing and presenting reports.
• Establishes and maintains professional social work standards for clinical assessments, treatment plans, and case documentation.
• Interprets clinical information and prepares comprehensive reports supporting recommended courses of action.
• Develops training curricula and presents on social work concepts to educate other professional disciplines.
• Trains staff in agency policies, procedures, and appropriate social work techniques.
• Ensures compliance with accepted social work practices, government regulations, and reporting requirements.
• Participates in conferences and meetings with supervisory staff, clinical professionals, and community organizations.
• Analyzes population health issues and serves as a liaison to community agencies.
• Provides clinical supervision and professional expertise to multiple social work units on complex cases.
• Coordinates staff work with other agencies to facilitate effective working relationships and enhance patient experience.
• Plans, develops, implements, and coordinates programs and activities to meet identified needs.
• Provides consultation and technical assistance, researching and recommending corrective actions.
• Assesses staff training needs, develops lesson plans, and conducts training sessions or workshops.
• Interprets and communicates procedures, regulations, and program directives.
• Responds to inquiries from patients, agencies, and courts, attending interdisciplinary meetings as needed.
• May participate in drafting policy and procedural manuals or instructional guides.

Requirements

• Valid license as a Licensed Clinical Social Worker (LCSW) issued by the New York State Education Department (NYSED).
• One (1) year of supervisory experience utilizing casework, group work, and/or community organization methodologies.

Department Preferences

• Strong Clinical Assessment Skills: Ability to evaluate and assess if a patient is at risk in the community due to suicidal or homicidal ideation, delusional thinking, or hallucinations. Includes decision-making authority to call 911 for emergency evaluation.
• Provides formal weekly individual supervision as per ACT regulations, as well as informal supervision to discuss symptoms of serious mental illness and appropriate interventions.
• Managerial/Administrative Skills:
  • Experience managing within a fast-paced environment while making sound clinical decisions.
  • Ability to multi-task and maintain strong organizational skills to manage documentation and timelines required by OMH and DOHMH guidelines.
  • Strong team management skills for daily team meetings and client crisis situations.
  • Experience managing team service dollars budget and providing guidance on spending.
