Social Worker LCSW (Per Diem) - Palliative Care
About the role
This is a per diem/on call inpatient position supporting patients in Santa Rosa Hospital but will be supporting Healdsburg and Petaluma Valley as needed. Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Qualifications
- Master's Degree in Social Work, Psychology or Counseling from an accredited school (preferred); or Ph.D. in Social Work, Psychology or Counseling from an accredited school (preferred).
- Coursework/Training in family counseling and case management.
- California Clinical Social Worker License upon hire.
- 1 year of experience in a health care setting or related field.

Pay
Base pay range: $48.69 - $75.58 per hour. Additional compensation may be available, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
Per-diem, day shift.
Location
Providence Palliative Care-Santa Rosa, 121 Sotoyome St, Santa Rosa, CA. On-site.
