Social Worker LCSW (Per Diem) - Palliative Care

Role Overview This is a per diem/on call inpatient position providing psychosocial assessment, counseling, social work, and emotional support services to terminally-ill and palliative care patients and their families. The role involves collaborating with a team to identify issues, explore options, and support decision-making processes. Responsibilities Conduct psychosocial assessments for patients and families.Deliver counseling and emotional support services.Assist in problem-solving and exploring realistic options with patients and families.Work collaboratively within a healthcare team to ensure comprehensive patient care. Qualifications & Skills Master's Degree or Ph.D. in Social Work, Psychology, or Counseling (preferred from an accredited institution).Training in family counseling and case management.California Clinical Social Worker License upon hire.At least 1 year of experience in a healthcare or related setting.
