About the role

This is a part time (32 hours/week) position traveling to see patients in Clark County WA area and may float to East Portland as needed. The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.

Responsibilities

  • Independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
  • Incorporates concepts and theories associated with life span development, neurobiology, and behavioral change.
  • Collaborates with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).
  • Works closely with various departments throughout the PHS.
  • Works with all inpatient or outpatient populations.

Requirements

  • Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
  • Upon hire: Oregon Clinical Social Worker License
  • Upon hire: Washington Clinical Independent Social Worker License
  • 2 years relevant Social Work experience. May include CSWA experience.
