About the role
We are seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive biopsychosocial, spiritual assessment, diagnosis, and treatment for patients in a home health setting. The role involves collaborating with patients and support systems to develop evidence-informed, person-centered care plans across the care continuum in partnership with an interdisciplinary team. The LCSW will perform duties that respect patient self-determination, reduce health disparities, and adhere to ethical standards.
Responsibilities
- Conducting assessments and providing therapeutic interventions
- Developing and implementing individualized care plans
- Collaborating with multidisciplinary teams
- Supporting patient self-determination and health equity
Requirements
- Master's Degree in Social Work from an accredited or candidate CSWE institution
- Oregon Clinical Social Worker License
- At least 2 years of relevant social work experience, including CSWA experience
