About the role

We are seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive biopsychosocial and spiritual assessments, diagnosis, treatment, and advocacy for patients and their support systems.

Responsibilities

• Collaborate with patients and multidisciplinary teams to develop and implement evidence-informed, person-centered care plans.
• Respect patient self-determination, reduce health disparities, and adhere to professional ethical standards.
• Work independently in home health settings to deliver care where patients are most comfortable.

Requirements

• Master's Degree in Social Work from an accredited or candidacy status school.
• Oregon Clinical Social Worker License upon hire.
• At least two years of relevant social work experience.
