Social Worker LCSW - Home Health

About the role

We are seeking a Licensed Clinical Social Worker (LCSW) to provide comprehensive biopsychosocial/spiritual assessments, diagnoses, and interventions for patients in a home health setting. The role involves collaborating with patients and multidisciplinary teams to develop and implement evidence-based, person-centered care plans that respect patient self-determination and reduce health disparities. You will engage in advocacy, treatment, and ensure ethical practice in line with NASW standards.

Responsibilities

• Conduct comprehensive biopsychosocial/spiritual assessments for patients in a home health setting.
• Develop and implement evidence-based, person-centered care plans in collaboration with patients and multidisciplinary teams.
• Provide diagnoses and interventions that respect patient self-determination and reduce health disparities.
• Engage in advocacy and treatment while ensuring ethical practice aligned with NASW standards.

Requirements

• Master's Degree in Social Work from an accredited program.
• Oregon Clinical Social Worker License (LCSW).
• At least 2 years of relevant social work experience, including CSWA experience.
