Social Worker LCSW (Part-Time 24-Hours)

About the role

We are looking to hire a Part-Time Social Worker (LCSW) who will support our patient population on-site at our Trumbull Dialysis Center. The Social Worker will work a 24-hour week with a flexible schedule but is responsible for the entire census of patients.

Responsibilities

• Support patients in understanding their rights and responsibilities related to End Stage Renal Disease.
• Guide patients in managing the physical, mental, emotional, and financial demands of their condition.
• Build meaningful and long-term relationships with patients and their families in an outpatient setting.
• Work collaboratively with the patient’s core care team to ensure comprehensive support.

Requirements

• Master's Degree in Social Work (MSW) with a specialization in clinical practice from an accredited school of Social Work.
• Masters-level license (e.g., LCSW, LICSW), meeting all licensure requirements for your state.
• Intermediate computer skills.
• Demonstrated knowledge of government and private insurance programs.
• Excellent interpersonal and communication skills.

Qualifications

• Currently enrolled in a Master's program in Social Work.
• Working towards obtaining your LCSW certification.

Skills

• Strong interpersonal and communication skills.
• Ability to build and maintain strong relationships with patients and their families.
• Knowledge of government and private insurance programs.
• Experience in providing support to patients undergoing dialysis.
