Social Worker / Geriatric Care Manager (Hybrid)

Queens, NY | Hybrid | $62K–$75K + Benefits M-F Day Hours, Full time TAG MedStaffing is partnering with a well-established, mission-driven nonprofit to hire a Social Worker / Geriatric Care Manager. The organization is privately funded and has a long history of helping older adults stay safe, independent, and connected to their communities. This is a direct-hire role on a small, close-knit team of experienced social workers. MSW candidates with strong relevant experience will be considered.Experience in geriatric care management, case management, entitlements, or related human servicesWorking knowledge of Medicare, Medicaid, Social Security, senior housing, and community-based servicesStrong communication, advocacy, organizational, and documentation skillsAbility to work independently while staying engaged with a collaborative teamProficiency with Microsoft Office and electronic documentation systemsA valid driver’s license and a vehicle are required. Home visits cover NYC, Westchester, Long Island.
