Care Manager / Social Worker

About the role

We are seeking a full-time Case Manager to support our Emergency Department team, focusing on discharge planning, patient resource connection, and promoting positive outcomes.

Responsibilities

• Coordinating care from pre-admission to post-discharge
• Facilitating interdisciplinary communication and early intervention
• Evaluating outcomes using clinical, financial, and utilization data
• Advocating for patients to resolve issues within healthcare delivery
• Connecting patients with community resources
• Supporting families through crisis intervention and education

Requirements

• Bachelor’s Degree in Nursing or Social Work
• Unencumbered RN license (for Care Managers) or licensed Social Worker credential
• Basic Life Support (BLS) certification within six weeks (for Social Workers)
• Strong communication, critical thinking, and problem-solving skills
• Ability to work in diverse healthcare settings and collaborate with multidisciplinary teams
