Care Manager / Social Worker

About the Role

Full-time Case Manager supporting Emergency Department operations, focusing on discharge planning, patient resource connection, and ensuring positive outcomes.

Responsibilities

• Coordinate patient care from pre-admission to post-discharge, promoting interdisciplinary communication and early intervention.
• Facilitate discharge planning and connect patients with community resources.
• Act as a client advocate, resolving issues within the healthcare delivery system.
• Support crisis intervention, educate families on medical situations, and coordinate healthcare programs.
• Participate in care conferences, transitional rounding, and collaborate with healthcare teams and community agencies.
• Provide care coordination and social services, including crisis intervention and resource referrals.
• Collaborate with healthcare teams and community stakeholders to promote patient safety and well-being.
• Participate actively in organizational and patient-centered initiatives.

Requirements

• RN license with a Bachelor's Degree in Nursing or completion within 18 months of hire.
• Social Worker: Bachelor's degree in Social Work; Licensed Social Worker (LSW) or equivalent credential preferred.
• BLS certification within six weeks of employment.
• Strong communication, critical thinking, problem-solving, and independent judgment abilities.
• Experience in structured and unstructured healthcare settings, with a focus on care integration and patient advocacy.
