About the Role

A full-time Case Manager supporting the Emergency Department, focusing on discharge planning, connecting patients with community resources, and ensuring positive outcomes.

Responsibilities

• Coordinate care from pre-admission to post-discharge.
• Facilitate interdisciplinary communication.
• Evaluate outcomes using clinical and utilization data.
• Advocate for patients within the healthcare system.
• Provide crisis intervention and assist families in understanding medical situations.
• Develop care plans by accessing social and health services.
• Facilitate communication among healthcare teams.
• Manage referrals and participate in care conferences and transitional rounding.
• Support patient flow and promote early intervention.
• Address client needs and collaborate with healthcare providers and community agencies to optimize patient care and safety.

Requirements

• For Care Managers: Bachelor's Degree in Nursing and an active RN license.
• For Social Workers: Bachelor's Degree in Social Work and BLS certification within six weeks.
• Licensure or credentials as per state regulations are required.
