Responsibilities
- Assessing Care Management members' social, emotional, and environmental needs
- Developing participants' social plans of care
- Negotiating community health services required under the plans of care
- Ongoing monitoring and re-assessment of Care Management members
- Traveling to locations within the VIVA HEALTH service area through a reliable means of transportation insured following Company policy
Requirements
- BSW
- Three years in social work
- Current LBSW license in good standing in the State of Alabama
- Valid Driver's license in good standing
PREFERRED
- MSW
- One year of social work experience in the field of community resource coordination
- Current LMSW license in good standing in the state of Alabama
- Certified Case Manager (CCM)

Qualifications
Must have a BSW with three years of social work experience, current LBSW license in good standing in the State of Alabama, valid Driver's license in good standing, and may require significant face-to-face member contact with duties regularly performed away from the principal place of business.
