Social Worker - Eddy Visiting Nurse & Rehab - Per Diem - Flexible Schedule

About the role

Eddy Visiting Nurse & Rehab Association is seeking a Per Diem Medical Social Worker to manage a caseload of patients requiring social work support through in‑home visits, with additional virtual or phone‑based support as needed. The position offers a flexible work schedule, allowing you to start and end your day from home, and provides the opportunity to be on the front line of transitions from inpatient settings to community‑based home health care.

Responsibilities

• Manage a caseload of patients in their homes, providing social work support.
• Collaborate with an interdisciplinary care team of nurses, therapists, and aides.
• Conduct comprehensive assessments of patient and family needs.
• Link individuals to community resources to enable them to remain at home or transition to a higher level of care when necessary.
• Provide virtual or phone‑based social work support in place of, or in combination with, in‑home visits.
• Work independently with a flexible schedule, including evenings and weekends.
• Participate in the transition of care from inpatient settings to home health care.

Requirements

• New York State Social Work Licensure (MSW/LMSW).
• Minimum of 1 year relevant experience in home care, the medical field, work with seniors, or related settings.
• Completion of Eddy VNA’s comprehensive orientation to home health care.
• Access to social work supervision and in‑depth computer training.
• Assistance obtaining insurance authorizations.
• Support staff available by phone for additional assistance.

Pay

• Hourly rate: $30.00 – $43.08, based on experience, skills, and education.
• Exempt positions under the Fair Labor Standards Act will be paid at a base‑salary equivalent of the stated hourly rates.
• Pay range may vary by location.

Schedule

• Part‑time, rotating shift.
• Flexible schedule with availability for evenings and weekends.
