Social Worker - Eddy Visiting Nurse & Rehab - Per Diem - Flexible Schedule

About the role

Per Diem Medical Social Worker (MSW/LMSW) – Home Care with a flexible schedule, evenings & weekends available. The role involves managing a caseload of patients requiring social work support through in‑home visits, as well as virtual/phone based support when needed.

Responsibilities

• Manage a caseload of patients in their homes, coordinating with an interdisciplinary care team of nurses, therapists, and aides.
• Conduct comprehensive assessments of patient and family needs.
• Link individuals to community resources to enable them to remain at home or transition to a higher level of care when necessary.
• Provide virtual or telephone social work support in place of, or in combination with, in‑home visits.
• Work independently with a flexible schedule, starting and ending the day from home.
• Serve on the front line of transitions from inpatient settings to community‑based home health care.

Requirements

• New York State Social Work Licensure (MSW or LMSW).
• Minimum of 1 year of relevant experience in home care, the medical field, work with seniors, or related settings.
• Ability to conduct assessments, coordinate care, and connect patients with community resources.

Pay

Hourly rate range: $30.00 – $43.08, based on experience, skills, and education. Exempt positions under the FLSA will be paid at the equivalent base salary.

Schedule

Part‑time, rotating shift with flexible evenings and weekends availability.
