Social Worker Case Manager (PRN)

Inova Loudoun Hospital is seeking a dedicated Social Worker Case Manager 1 to join the Case Management Team on a PRN basis (3 weekend nights per month). About the Role

The Social Worker Case Manager I (PRN) evaluates patients' ability to progress throughout the continuum of care. This role involves collaboration with physicians, nursing, and other multidisciplinary care team members to ensure timely and appropriate patient management. The position requires a working knowledge of utilization management, managed care, and payer issues, along with providing discharge planning and continuity of care for assigned patients in acute and post-acute settings.

Responsibilities

• Participates in the assessment of patients' biopsychosocial needs through review of patient information, personal contact with patients/families, and interdisciplinary care team members.
• Communicates routinely with patients, families, interdisciplinary care team members, and other appropriate parties regarding the status of patients' care plans, progress toward treatment goals, identification of concerns/problems, problem-solving, and conflict resolution.
• Ensures all options available to support a successful transition and critical elements of patients' care plans are communicated and documented to ensure continuity of care.
• Refers cases and issues appropriately to resolve barriers to care progression and acts as an advocate for patients.
• Assesses psychosocial risk factors of patients/families through evaluation of prior functional levels, support systems, reactions to illnesses, and coping abilities based on preliminary risk screenings.
• Intervenes with patients/families regarding emotional, social, and financial consequences of illness and/or disability.
• Serves as a resource person and provides counseling related to treatment and end-of-life decisions, advocating for patient/family empowerment and independence.
• Provides discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Initiates and facilitates referrals to clinics, home healthcare, hospice, skilled nursing facilities (SNF), acute rehab, long-term acute care (LTAC), transitional care management (TCM), and medical equipment/supplies as indicated.
• Collaborates with the interdisciplinary care team, patients, and families in assessing and coordinating discharge planning needs, delivering post-discharge services, and transitioning patients from hospital to discharge settings and ongoing community care.
• Documents relevant discharge planning information in the medical record according to department standards and care management plans.
• Collaborates and communicates with internal/external case managers.
• Provides coordination of services and acts as a key liaison between patients, families, and interdisciplinary healthcare members.
• Performs other duties as assigned.

Requirements

• Master's Degree in Social Work (MSW).
• 1 year of experience in clinical care or clinical case management.
• Basic Life Support certification (upon start).

Preferred Qualifications

• One year of previous inpatient (hospital) case management experience and case management discharge planning.
