Case Manager Social Worker - PRN

About the role

Provide psychosocial assessment, patient advocacy, and discharge planning for high-risk inpatients in collaboration with the clinical team and medical providers. Coordinate complex discharge arrangements such as skilled nursing facilities (SNF), long-term care (LTC), and hospice, and connect patients and families to appropriate hospital and community resources.

This PRN role supports staffing needs during absences and focuses on promoting patient and family wellness, care outcomes, and resource access.

Responsibilities

• Perform psychosocial assessments for high-risk inpatients
• Provide patient advocacy within the clinical team
• Develop and implement discharge plans
• Coordinate complex discharge arrangements (SNF, LTC, hospice)
• Connect patients and families to hospital and community resources
• Collaborate with medical providers and multidisciplinary teams

Requirements

• Master’s degree in Social Work
• LMSW credential (LCSW preferred)
• 3-5 years of relevant experience
• Active Maryland Social Work License
• Effective verbal and written communication skills
