About the Role
The Inpatient Social Worker provides advocacy, discharge planning, and psychosocial support for high-risk hospitalized patients, collaborating with clinical teams and medical providers to enhance patient and family wellness and access to community resources.
Responsibilities
- Coordinate complex discharge plans, including SNF, LTC, and hospice referrals.
- Assist patients and families with psychosocial interventions and resource linkage.
- Support patient outcomes by promoting wellness and care access.
Requirements
- Master’s degree in Social Work required; LMSW certification essential, LCSW preferred.
- 3-5 years of relevant experience.
- Valid Maryland Social Work License based on education level.
- Effective verbal and written communication skills and ability to follow instructions.
