About the Role
Provide psychosocial support, discharge planning, and patient advocacy for high-risk hospitalized patients.
Responsibilities
- Collaborate with clinical teams and medical providers to coordinate discharge plans, including SNF, LTC, or hospice.
- Deliver psychosocial interventions to promote patient and family wellness.
- Assist in access to hospital and community resources to improve care outcomes.
- Fill in for staff absences, providing support during leaves and vacations.
- Provide complex discharge planning, psychosocial interventions, patient and family advocacy, and collaborate with healthcare teams to enhance care outcomes for patients with complex health needs.
Requirements
- Master’s in Social Work (MSW) required; LMSW license mandatory; LCSW preferred.
- 3-5 years of relevant experience.
- Knowledge of multiple disciplines and excellent verbal and written communication skills.
- MD Social Work License based on education level.
