About the role
The Social Worker (MSW) provides non-clinical, biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. Responsibilities include crisis intervention, patient/family support, high-risk screening, financial-assistance referrals, and post-acute care placement, with a focus on behavioral health and resource needs. The role emphasizes patient- and family-centered care, self-determination, and compassionate problem-solving across intensive care units, Trauma, Oncology, NICU, Transplant, and the ED. The MSW responds to physician and non-discharge-planning consults facility-wide.
Responsibilities
- Collaborate with RN case managers, nurses, physicians, insurers, and staff to proactively identify and address patient needs.
- Identify and resolve discharge and recovery barriers (financial, emotional, psychosocial, medical, spiritual).
- Maintain working knowledge of Medicare, Medicaid, and private-payer regulations and processes.
- Develop discharge plans for high-risk patients with psychosocial or discharge-planning needs.
- Coordinate and facilitate care-team meetings with patients, families, and caregivers.
- Assist patients and families in making difficult decisions consistent with their preferences and values.
- Provide psychosocial counseling and support for chronic, acute, or terminal illness.
- Deliver consultation, education, counseling, advocacy, and referrals to address emotional, social, and financial aspects of care.
- Offer emotional support during complex or emotionally charged situations, including end-of-life discussions, terminal-illness notifications, death notifications (with physician), and crisis interventions.
Requirements
- Master’s degree in Social Work (LMSW/RCSW-I, Licensed Master Social Worker/Registered Clinical Social Work Intern, or LBSW Licensed Baccalaureate Social Worker).
