Social Worker
About the role
As a Social Services Designee you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at our facility by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Qualifications
- High school diploma or equivalent (minimum)
- Two (2) years of experience as a Social Services Designee in a Skilled Nursing Facility (preferred)
- Bachelor's degree in Social Work or similar field (preferred)
- Five (5) years of related experience (preferred)
