About the role

Lions Rehab Center, a beautiful 101-bed skilled nursing facility in Cumberland, Maryland, is seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing social work services to our residents and patients, focusing on their physical, emotional, and social well-being.

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct social services programs of the facility
  • Develop, administer, and coordinate department policies and procedures
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator
  • Participate in community planning related to interests of the facility and services and needs of residents and family
  • Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting them as required
  • Make written and oral reports/recommendations to the Administrator and relevant department heads concerning the operation of the social service department
  • Be prepared to handle emergencies as they arise (i.e. rescheduling work assignments and work)
  • Orient residents and family to the Nursing Center, helping both adjust to the new environment
  • Develop a social history on each resident
  • Develop social assessments and care plans; which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
  • Answer inquiries of admission and provide tours to prospective family members/residents
  • Be actively involved in pre-admission and admissions processes
  • Assist in continually updating the admissions waiting list
  • Interview residents and family members as necessary and in private settings
  • Ability to listen and act compassionately with residents' and family members; must be able to work on own and maintain related jobs

Requirements

  • Bachelor's degree from an accredited school of Social Work or related field is required
  • Must possess any certifications/licensures as required by the State of employment to practice in long-term care
