Position Summary: The Social Worker assists eligible clients and their caregivers in gaining access to needed medical, social, educational, and other services necessary for appropriate care and treatment. Additionally, may provide therapeutic interventions to the child and their family in the form of rehabilitative services. Primary Responsibilities Provides comprehensive coordination and implementation of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e. DHS, COA). Complies with local office procedure for referral and service delivery. Completes a systemic assessment of the needs of the client and family. Establishes service parameters with family or legal custodian as client’s needs dictate. Provides complete and required documentation, within established timelines relating to case management and/or rehab services. Implements treatment services to meet the needs and goals of the clients and families utilizing quality case management practice to provide for the best interest of the client and family. Delivers skill development and therapeutic services for assigned individual and family cases and assist them in maintaining a stable environment, and in developing the values, principles, and skills necessary to create a healthier family system. Creates a safe and trauma-informed therapeutic environment in which treatment interventions can be implemented. Assist families in crisis intervention situations as they may arise. Provides direction to other team members when applicable and make recommendations for discharge, finalization, and permanency. Advocates for the development and maintenance of funding and resources to meet youth and family needs at local, state, and national levels. Develops and maintains community contacts to promote the organization and collaborative efforts.Provides crisis intervention as needed to maintain safety and provide for the best interest of the client and family. Integrates trauma informed care principles into practice by sharing knowledge and understanding of trauma with the client, family, and relevant team members; utilizes and advocates for the use of trauma-sensitive strategies to support the youth and their family. Routinely screens for trauma exposure and related symptoms and demonstrates competence in using screening tool results to enhance service provision. License foster families according to established standards.Routinely makes resources available and shares knowledge with child, family, and other providers on trauma exposure, its impact, and trauma-focused treatment. Engages in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma. Emphasizes continuity of care and collaboration across child-service systems. Complete foster care home studies with families.Understands the potential for secondary traumatic stress and practices self-care strategies to mitigate these effects. Meets standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families and ensures documentation required and governing/accrediting agencies; maintain current files on all youth in placement.Produces informal and formal reports as requested by parents, referral agencies, courts, etc. Participates in staff and supervision meetings and required in-service meetings. Understands and supports the Nexus philosophy and mission. Participate in individual and group supervision including clinical consultation to review cases and ensure best practices are provided to all children and families. Provides timely and thorough information to supervisor regarding case incidence, service delivery struggles and difficult team processes. Ensures confidentiality of all treatment. Maintains a schedule that is adjustable and flexible to meet changing work needs and demands of clients and their families. Demonstrates the ability to handle emergency or crisis situations and use sound judgment by taking appropriate actions. Remains calm and effectively manage stressful workload conditions and difficult, emotionally charged or crisis situations. Utilizes organizational skills and tools to proactively manage multiple client needs/demands. Represents the organization positively within the community.
