Social Worker

About the Role

The Social Worker collaborates with the team to coordinate care for mental health, behavioral, and social needs. This role facilitates communication between patients, families, healthcare professionals, community and state agencies, and other interdisciplinary team members. The Social Worker assists with implementing care plans and coordinating behavioral health and social services.

Responsibilities

• Functions as a member of a professional team providing evaluation and treatment for psychiatric patients, including psychological assessment, crisis intervention, case management, patient advocacy, care coordination, and community outreach.
• Provides input in treatment decisions and presents specific treatment plans for approval.
• Obtains information from patients and other appropriate sources to initiate treatment recommendations; records all information in the medical record.
• Involves patient, family, and others as appropriate in the delivery of care and treatment recommendations.
• Works with patients to determine goals to be achieved during treatment.
• Makes arrangements for the safe and expedient transfer of patients as required.
• Acts as a resource of information for team members, patients, and referral sources.
• Assists with provider schedule maintenance, sets up appointments, and arranges follow-up treatment.
• Supports quality patient care, patient satisfaction, and assists in accomplishing related goals.
• Assists with insurance verification and authorization procedures.
• Ensures patient progress from acute care through post-discharge as part of an interdisciplinary team, focusing on quality, efficiency, and cost-effectiveness.
• Interacts with patients, family members, healthcare professionals, and community/state agencies, serving as a liaison for clinical and referral information.
• Identifies high-risk patients from psychosocial/financial perspectives, assesses psychological needs, and provides support, counseling, care management, and referrals.
• Resolves psychosocial patient care issues and develops/implements complex patient transition/discharge plans.

Work is typically performed in a clinical environment.

Requirements

• Master's Degree in Social Administration (MSSA) (Required)
• Master's Degree in Social Work (Required)
