Social Worker
Job Summary
You will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
Responsibilities
- Conducts psychosocial assessments and develops care plans
- Provides interventions and counseling for emotional, social, and resource issues
- Serves as the professional on the multidisciplinary team advocating for the patient's perspective in care planning
- Maintains current clinical training and skills to provide competent care to various patient populations
- Manages caseload with timely response to referrals and high-risk screening
Requirements
- Master’s degree from a School of Social Work accredited by the Council of Social Work Education
- One year of clinical and/or medical social work experience
