Social Worker- Adult Daycare

Responsibilities

• Assess, counsel, interview, and educate clients and their family members.
• Assist in planning, organizing, and implementing facility policies and procedures to meet the emotional and social needs of patients/residents.
• Interview and review personal histories to assess patients’ situations.
• Plan, coordinate, manage, and implement support packages to help patients/residents address difficulties and overcome dependencies.
• Develop and execute individual treatment plans, including applying Medicaid and Medicare on behalf of patients.
• Assist patients with Food Stamps, housing applications, and phone assistance.
• Interview service users to assess current condition, needs, strengths, and weaknesses.
• Set tailored, measurable goals for each case and monitor progress, modifying treatment plans as needed.
• Provide information and counseling on optimal courses of action during sessions.
• Facilitate referrals to other healthcare professionals and programs.
• Maintain accurate patient documentation.
• Act as a key worker and cooperate with multidisciplinary teams.
• Support the facility and promote patient/resident rights, privacy, dignity, and well‑being in a safe environment.
• Participate in teamwork and comply with the Code of Conduct.

Qualifications

• BS in Social Work (required).
• Social Work certification to practice in New York State.
• 1–2 years of experience working with geriatric populations.
• Experience in nursing and rehabilitation settings (new graduates with relevant experience are welcomed).

Pay

Salary: $60,000 – $70,000 per year.

Schedule

Full‑time, 8 am – 4 pm, no weekends.
