About the Role

Valley's Adult Outpatient Valley West treatment program provides flexible, evidence-based mental health and substance use services that help individuals build healthier lives while remaining connected to their families, work, and communities. As a Social Service Worker, you'll play a key role in supporting client progress through therapeutic behavior management, care coordination, skill development, and ongoing support. This position offers the opportunity to build meaningful relationships and help clients achieve lasting growth and recovery.

Responsibilities

  • Provides therapeutic behavior management as prescribed by care plan
  • Assists therapist as needed to develop and review safety/crisis plans and care plans
  • Develops therapeutic relationships with clients
  • Reviews and assesses care plans
  • Coordinates services among internal/external agencies and internal/external providers
  • Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
  • Ensures continuity of care
  • Meets required productivity standards and other organizational expectations

Requirements

  • Bachelor’s degree in Social Work or related field
  • Internship experience as required for SSW license
  • SSW licensure in state of practice
  • CPR certification
  • Valley de-escalation certification

Pay

Range starts at $23.00/hour (pay is calculated based on years of licensed experience).

Schedule

Monday-Friday, 9:00am-6:00pm (1 day/wk will need to work until 8 pm). Clinic lunch is at 12:30pm every day.
