Responsibilities
- Complete psychosocial assessments and identify residents’ emotional, social, and psychological needs
- Develop and implement individualized care interventions that support resident goals and quality of life
- Provide support and guidance to residents as they adjust to the nursing home environment
- Advocate for resident rights, dignity, independence, and personal choices
- Assist residents and families with accessing appropriate community resources and services
- Participate as an active member of the interdisciplinary care team
- Collaborate with nursing, rehabilitation, dietary, administration, and other departments to support resident-centered care
- Participate in care planning meetings and quality improvement initiatives
- Aid in discharge planning and coordination of appropriate community services
- Provide education and support to residents, families, and caregivers regarding available resources and services
- Maintain accurate and timely documentation related to resident assessments, interventions, and progress
- Ensure compliance with applicable state and federal regulations and professional standards
- Maintain confidentiality of resident information in accordance with HIPAA regulations
Qualifications
- Bachelor’s degree in Social Work or related field required (based on state requirements)
- Current Social Work licensure or certification required as applicable by state regulations
- Experience working with older adults, long-term care, rehabilitation, or healthcare populations preferred
- Knowledge of psychosocial needs, care planning, and resident rights
