Responsibilities

• Serves as the case manager for assigned patients.
• Collaborates with the interdisciplinary team to coordinate the assessment process.
• Completes psychosocial assessments within organizational guidelines.
• Conducts group and individual therapy sessions as needed.
• Develops and implements patient treatment plans based on thorough assessments.
• Demonstrates flexibility in scheduling to meet practice and patient needs.
• Maintains patient confidentiality while reporting suspected abuse or neglect.
• Documents all patient interactions, activities, and treatment plans accurately.
• Explains and discusses legal documents like living wills and durable power of attorney sensitively.
• Fulfills discharge planning requirements and ensures timely follow-up coordination.
• Participates in quality improvement initiatives and supports staff through counseling and support groups.
• Assists in the development and implementation of the interdisciplinary treatment plan.
• Promotes a therapeutic environment through formal and informal methods.
• Engages in professional development and lifelong learning to improve practice settings.
• Provides consultation and in-service education to medical and hospital staff.

Qualifications

• Master's Degree in Social Work (LMSW) from the Louisiana State Board of Social Work Examiners.
