About the role

This is an hourly position covered by the Ohio Health Care SEIU/1199 bargaining unit (union), with a pay range of 10 on the Ohio Health Care SEIU/1199 Pay Range Schedule. Normal working hours are 8:00 am – 4:30 pm. The position is located at the Northwest Ohio Behavioral Healthcare facility, 930 South Detroit Avenue, Toledo, OH 43614.

Responsibilities

• Coordinate assessments
• Develop and implement treatment plans
• Provide interventions with acute, more complex clients
• Maintain necessary documentation
• Complete and maintain client records
• Complete and maintain discharge documents
• Coordinate referrals
• Plan and implement continuities of care from hospital to community by functioning as a linkage for the treatment team with the case management system through ongoing regular collaboration

Qualifications

• Requires current license as a social worker (LSW) issued by the State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.28.
• Pursuant to ORC 4757.41(A)(5), any person engaging in social work or professional counseling as a civil service employee (as defined in ORC 124.01) who has at least two years of service as of July 10, 2014 is exempt from the LSW requirement.
• Official transcripts are required for all post‑secondary education, coursework, or degrees listed on the application; they must be submitted before a formal employment offer. Failure to provide transcripts within five (5) business days of request results in disqualification. Transcripts printed from the institution’s website are not accepted. The Ohio Department of Behavioral Health reserves the right to evaluate the academic validity of the degree‑granting institution.

Pay

Compensation is $27.48 per hour. The position begins at Step 1 of the pay range schedule, with a potential increase after six months of satisfactory performance and yearly raises thereafter, subject to any applicable union contract or Ohio Revised Code requirements.

Schedule

Full‑time schedule: 8:00 am – 4:30 pm, Monday through Friday.
