About the role

The PACE Social Work Supervisor position is responsible for the administrative oversight and supervision of direct reports at the assigned PACE site, which will include the master level social workers and other PACE team members as needed. Promotes effective collaboration and individualized care planning amongst interdisciplinary team members.

Responsibilities

• Coordinate routine assessments and care plans
• Assist with family conferences and community referrals
• Monitor team member documentation
• Onboard and orient new hires
• Conduct routine competency and work performance evaluations
• Provide guidance and support related to participants’ behavioral health concerns to ensure a person-centered approach
• Provide guidance related to advanced life planning and end-of-life care
• Utilize knowledge of state and federal regulatory requirements related to social work department tasks to provide effective oversight and ensure compliance
• Collaborate with other site leaders to monitor site safety, overall staffing changes, and program budgets
• Provide coverage of SW department roles as needed

Requirements

• Master's degree in social work (MSW) from an accredited university (required)
• LCSW (preferred)
• Minimum of 2 years' experience in a supervisory position in a health-related or human services field (required)
• Minimum of 2 years of experience in case management/social advocacy/care planning with the geriatric population (preferred)
• Bilingual Spanish/English, Chinese/English, or another second language (strongly preferred)
• Valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines (required)

Pay

$90,090.88 - $112,613.60 annually

Actual salary offers are considered by various factors, including budget, experience, skills, education, licensure and certifications, and other business considerations. The range is subject to change. This role may also qualify for discretionary bonuses or incentives.
