Overview

Dialysis Clinic, Inc. is recruiting for a Dialysis Social Work Administrative Assistant to support our nonprofit mission of providing individualized care for patients with chronic kidney disease. The role supports the licensed social worker in providing psychosocial services, care coordination, and administrative duties.

Responsibilities

  • Assist in providing emotional support to patients adjusting to dialysis treatment.
  • Reinforce education on treatment adherence, coping strategies, and lifestyle adjustments.
  • Observe and report changes in patient mood, behavior, or psychosocial status to the social worker.
  • Coordinate patient services including transportation, housing, financial assistance, and community resources.
  • Aid in preparing and distributing various documents and information such as monthly psychosocial assessment packages, patient contact & insurance updates, welcome packages for new patients, information for QAPI meetings, Patient Quality of Life Surveys, updating transplant lists, etc.
  • Input various patient information in Darwin - patient consents, history, healthcare proxy, etc.
  • Review patient Grievance and Complaint binder daily.
  • Cook up and assist the social worker with Lobby days and quarterly workshops for patients.
  • Maintain accurate and timely documentation in patient records per facility policy.
  • Schedule appointments, follow-ups, and patient outreach.
  • Track patient compliance and attendance issues (e.g., missed or shortened treatments).
  • Advocate for patient needs under the supervision of the licensed social worker.
  • Assist patients with navigating insurance, benefits (Medicare/Medicaid), and financial concerns.
  • Work closely with interdisciplinary team members including nurses, dietitians, and physicians.
  • Participate in care planning meetings and contribute relevant observations, as needed.

Qualifications

  • Strong communication and interpersonal skills.
  • Organizational and documentation skills.
  • Ability to maintain confidentiality (HIPAA compliance).
  • Empathy and cultural sensitivity.
  • Basic knowledge of community resources.

Education/Training:

  • High school diploma required.
  • Associate’s or Bachelor’s degree in Social Work, Human Services, or related field preferred.
  • Prior experience in healthcare, dialysis, or social services preferred.
  • Prior experience working with chronically ill or vulnerable populations is a plus.
