About the role

The Quarters at Des Peres seeks compassionate and thoughtful individuals to join our team. We partner with skilled nursing centers to support a small team of dedicated professionals with the resources of a larger enterprise.

Essential Functions

  • Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.
  • Develop specific, measurable, realistic plans and objectives, which enhance social service daily.
  • Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families.
  • Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning. Include family in teaching as appropriate.
  • Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights and the “Residents’ Bill of Rights”.
  • Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
  • Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
  • Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and center policy is followed.
  • Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
  • Be flexible, organized, and function under stressful situations.
  • Maintain a good rapport and working relationship with medical staff, departments, and employees.
  • Manage and operate equipment safely and correctly.
  • Treat patients and their families with respect and dignity.
  • Support and maintain a culture of safety and quality.

Qualifications

  • Bachelor’s degree in social work is preferred or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
  • One year of supervised social work experience in a health care setting working directly with individuals is preferred.
  • Current State Clinical Social Work Licensure.
  • Ability to work cooperatively as a member of a team.
  • Ability to communicate effectively with residents and their family members, and at all levels of the organization.
  • Ability to react decisively and quickly in emergency situations.
  • Ability to maintain confidentiality.
  • Knowledge of training techniques for clinical staff.
  • Good Ethical Practice.
