Social Services Director
Outfield Healthcare Partners is seeking a dedicated and compassionate Social Services Director (LSW) to join our leadership team in Plainfield, Indiana. In this role, you will make a meaningful impact every day by supporting residents and families through care transitions, discharge planning, emotional support, advocacy, and quality-of-life services within our skilled nursing facility.

Responsibilities
- Lead and manage the Social Services department in compliance with federal, state, and company guidelines
- Complete social histories, psychosocial assessments, care plans, and required documentation
- Coordinate discharge planning and community resource referrals
- Support residents and families with transition planning, emotional support, and resident rights advocacy
- Participate in interdisciplinary care plan meetings and quality assurance initiatives
- Assist residents with Advance Directives and psychosocial needs
- Collaborate with hospitals, hospice agencies, outpatient providers, and community organizations
- Maintain accurate documentation within PointClickCare and other systems
- Educate residents, families, and staff on resident rights, abuse prevention, and psychosocial care
- Prepare the department for state survey readiness and compliance
Requirements
- Licensed Social Worker (LSW) in the State of Indiana
- Bachelor’s Degree in Social Work or Human Services-related field
- Minimum 1 year of supervisory social work experience in a healthcare setting
- Experience working with geriatric residents
- Strong understanding of discharge planning, care plans, and psychosocial assessments
Preferred Qualifications
- Skilled Nursing Facility (SNF) or Long-Term Care experience
- Experience with PointClickCare or electronic medical records
- Knowledge of Medicare, Medicaid, PASRR, and state/federal regulations
