Social Services Designee
About the role
The Social Services Designee coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. You will act as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively. The Social Services Designee coordinates the overall psychosocial care plan for residents. The Social Services Designee provides support and advocates for resident/patient and their family. The Social Services Designee will develop and maintain community resources for residents.
Responsibilities

Requirements
- Related coursework in social work or psychology
- Hospital or nursing home experience, preferred
- Able to read, write and communicate in English

Pay
$23.34 - $34.72 per hour
