Position

Reports To: Director of Social Services

Key Responsibilities

  • Craft and document personalized care plans for residents' social and emotional well-being.
  • Collaborate with various departments to ensure effective discharge plans.
  • Knows and complies with Residents’ Bill of Rights.
  • Maintains updated community resources for residents and families.
  • Addresses reported concerns, provides resolutions, and ensures proper documentation.
  • Easy accessibility to residents, families, and staff for support and guidance.
  • Provides timely referrals to appropriate agencies for assistance to the resident/family (i.e. conservatorship application, Medicaid application, etc).

Requirements

  • Bachelor’s Degree from an accredited program in Social Work or related field.
  • Must be knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as be familiar with residents’ rights and documentation requirements in accordance with current regulations and/or guidelines.
  • Attention to detail, good follow through skills and ability to prioritize multiple tasks.
  • Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus.
  • Must have a vehicle and a valid drivers’ license.
  • Demonstrates cooperation in assisting others and ability to work as a team.
  • Patience, tact, enthusiasm and a positive attitude toward the residents and their support networks.
  • Able to remain calm in stressful situations.
  • Able to communicate with residents, staff and family members in a professional manner.
