Social Services Designee

About the role

Responsibilities

• Aid in preparing residents for their future.
• Ensure residents' social and emotional needs are met during their stay.
• Congnorate resident discharges and participate in resident care conferences.
• Ensure residents are comfortable and safe.

Requirements

• High school diploma or equivalent with 2 years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility, a bachelor’s degree in Social work or similar field, and 5 years of related experience is preferable but not required.

Qualifications

• High school diploma or equivalent.
• Two (2) years of experience in a Social Worker role, preferably in a healthcare setting.

Skills

• Knowledge of social and emotional needs of residents.
• Attention to detail.
• Ability to coordinate resident discharges and participate in resident care conferences.
