Social Services Designee

About the role

Responsibilities

• Assist in preparing our residents for their future.
• Ensure our residents' social and emotional needs are met during their stay.
• Congnorate resident discharges and participate in resident care conferences.
• Ensure our residents are comfortable and safe.

Requirements

• As a minimum, a high school diploma or equivalent and two (2) years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior Social Services Designee experience in a Skilled Nursing Facility, bachelor’s degree in Social work or similar field and five (5) years of related experience is preferable but not required.
