Social Services Designee
Position Summary
The Social Services Designee supports residents, families, and facility staff throughout the admission and discharge process while serving as a key liaison to community agencies and referral sources. This role manages referrals, tours, screenings, admission paperwork, and discharge planning—including arranging home health services and equipment—and provides emotional, informational, and adjustment support to residents and families. As part of the interdisciplinary care team, the Social Services Designee conducts psychosocial assessments, maintains documentation, participates in care planning, and ensures each resident’s individual needs are addressed to help them achieve the highest level of functioning possible. The position also clarifies legal documents, fosters positive relationships with referral partners, and promotes a respectful, inclusive environment.
Qualifications
- Pyschosocial Understanding: Must recognize and effectively address the emotional and psychosocial needs of residents.
- Core Competencies: Strong communication, organization, leadership, and management skills are essential.
- Preferred Experience: Background in healthcare settings such as skilled nursing facilities or assisted living communities is desirable.
- Interpersonal and Adaptive Skills: Must be personable, trustworthy, a strong listener, fluent in English, and able to navigate fast-paced, ambiguous environments with diverse populations.
