Social Services Designee

About the role

As a social worker at Atrium Centers, you will play a crucial role in enhancing the well-being of residents through the development, implementation, and evaluation of social service programs. Your work will be guided by state and federal guidelines, and you will collaborate closely with residents, families, and interdisciplinary teams to meet their emotional, psychosocial, and social needs.

Responsibilities

• Develop, plan, implement, and evaluate social service programs in accordance with state and federal guidelines
• Identify and address residents’ social, emotional, and psychological needs
• Provide service coordination for residents and families
• Support care planning and interdisciplinary collaboration
• Ensure compliance with applicable regulations and documentation standards

Requirements

• Licensed Social Worker preferred, but not required
• At least one year of supervised social work experience in a healthcare setting (elderly psychosocial experience preferred)
• Familiarity with nursing home regulations, including MDS procedures
• Familiarity with Medicare and Medicaid reimbursement and eligibility guidelines
• Strong written and verbal communication skills
• Positive, can-do attitude with excellent attendance
