Social Services Coordinator - Team Care Management

About the role

The Social Services Coordinator plays a key role in helping patients and families access the resources and support they need to live healthy, safe, and independent lives. As part of an interdisciplinary care team, this position works closely with providers, nurses, behavioral health professionals, pharmacists, and care managers to address social determinants of health and remove barriers to care.

Responsibilities

• Conducting patient assessments
• Connecting individuals with community resources
• Aiding with insurance and financial assistance programs
• Supporting care transitions
• Facilitating advance care planning
• Empowering patients to achieve their health goals through education and self-management support
• Collaborating with community agencies and healthcare teams to ensure patients receive the appropriate level of care and support

Qualifications

• Education: Bachelors of Social Work required
• Licenses/Certifications/Registries: BSW required
