Responsibilities
- Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits.
- Orient and assist new residents in adjusting to the facility.
- Complete MDS forms in a timely fashion.
- Assist residents and families with adjustment to changes, including room changes.
- Assist families with the engraving and/or marking of residents’ personal items.
- Review residents’ rights with residents and staff and attend to various personal needs of the residents.
- Aid in the development of a volunteer program and facilitate family group meetings.
- Document progress notes that relate to each resident’s care plan when necessary and within policy timeframe.
- Comply with all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective back supports.
- Understand, comply with and promote all rules regarding resident rights, promote positive relationships with residents, visitors and regulators, to include presenting a professional appearance.
- Other duties as assigned or needed.
Skills
- Proven work experience in long term care.
- Working knowledge of social theories and practices.
- Social perceptiveness and empathy.
- Ability to build and maintain professional helping relationships.
- Ability to relate and communicate with diverse population and groups.
- Resilience along with ability to assess situations.
- Willingness to submit to background checks.
- Psychology or sociology experience preferred.
