Social Services Coordinator, Housing Case Manager PT

About the role

Part-time position, Monday through Friday, coordinating home and community-based services to residents and families assigned to a senior apartment building.

Responsibilities

• Conduct needs assessments, crisis intervention, and case management for residents while maintaining confidentiality.
• Serve as a resource for the Housing Manager and maintain community contacts.
• Arrange educational programs for residents and staff.
• Maintain files in accordance with HUD and LSSI requirements and complete all reports accurately and in a timely manner.
• Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, workforce members, and collaborative organizations with dignity, respect, honesty, and fairness.

Position Qualifications

• Bachelor’s degree in human services or related field required.
• Background check clearance required.
• Experience working with older adults and entitlements preferred.
• Demonstrated ability to communicate clearly, both verbally and in writing.
• Excellent organization, presentation, and PC/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) and related software.
• Valid driver’s license in good standing for the state of residency required.
• Access to reliable transportation required.
• Valid IL statutory minimum liability insurance coverage, including bodily injury and property damage required.
